In Spring 2016, a number of local organizations supporting residents with chronic illnesses launched a coordinated access pilot for Chronic Disease Prevention and Management (CDPM) programs and services. The pilot involved an online directory of available services and secure, electronic referral. One of the goals was to reduce system navigation challenges by making it easy for referrers to find and connect patients to services that will assist them to manage their chronic disease, or to prevent the onset of chronic disease.
Another major goal was to inform the development and roll-out of a region-wide, integrated patient referral portal that is currently in development. The portal will eventually provide access to a broad range of programs and services across the health system.

- Improved awareness of the broad range of CDPM services available across Waterloo Wellington.
Web-based referral access, providing the ability to easily and quickly refer patients to the appropriate local service that can meet their patient’s particular need.
The ability to improve the health outcomes of residents through timely referral to supportive services
