Orthopedic Surgery

The SCA program is currently working with individual groups of service providers on the development of a coordinated approach to access (through either coordinated referral mechanisms or centralized intake centers, or both).

On May 16, 2016, as part of the foundation for the Proof of Concept (see below), the Waterloo Wellington Regional Coordination Centre (WWRCC) began supporting referrals to orthopedic surgeons for all primary care providers in Waterloo Wellington.
Key activities include:
  • Creation of a common orthopedic referral form to improve the quality of referrals.
  • Creation of a Regional Coordination Centre (Central intake) to reduce system navigation challenges and act as a single point of contact to process Orthopedic referrals
  • Creation of specialists’ directory to identify what Orthopedic Surgeons practice in the region and their subspecialties.
  • Creation of a Medical Specialist database to improve communication, ensure that referrals are tracked and no patient is lost in the system.

The SCA program is moving forward with early adoption of the Ocean eReferral Network for orthopedic referrals in Waterloo Wellington. We are suppporting Think Research Consortium (TRC) in the development of this tool, which will allow information to flow securely and be distributed across the continuum of care. The Proof of Concept (POC) will focus on the integration of three specific pathways: CDPM, Orthopedic Specialists, and Diabetes Central Intake.
